Voyage Care - care provider

MyPAd lets you write a social prescription for patients, customers or family members in Wiltshire. The groups, services, and information pages in the site can be saved to build your network of support in your community. Save links to information you have found on YourCareYourSupportWiltshire. Add your own notes and update them whenever you wish. If you are logged onto your account in MyLife, MyPad will save your links and notes until you delete them. If you are not logged on, the links and notes are saved until you close down YourCareYourSupportWiltshire.

Voyage Care offers person-centred, high quality and enabling support for people to live the life they choose. We have 30 years of experience in providing specialist and expert support for people with a range of needs, including: 

  • Learning disabilities 
  • Physical disabilities 
  • Autistic Spectrum Conditions 
  • Brain injury rehabilitation and neurological conditions 
  • Mental health conditions 
  • Young people in transition – preparing for adulthood 
  • Challenging behaviour 
  • Nurse led care for children and adults 
  • Other complex care needs 

We support people to achieve their independent living outcomes in a range of settings including: 
  • Supported living 
  • Care and support at home and to access the community 
  • Community day opportunities – accessing local activities individually or as a group 
  • Residential care homes 
  • Respite and short breaks 
  • Intensive support 

The people we support are at the heart of all we do and we’re committed to providing exceptional, specialist care and support.

We provide flexible and responsive support, ranging from a few hours a week to 24 hours a day. Together, we co-create a personalised support plan to reflect what is important to the person enabling people to live as independently as possible and achieve their own goals. 

We can support people achieve a wide range of goals and outcomes in a way that they choose. 
These outcomes may include:

  • Taking part in your community: accessing activities in your local area.

We are experienced in supporting people funded through Personal Budgets, Personal Health Budgets and Direct Payments. We support people to explore creative ways to maximise their funding and enjoy choice and control.

If people accessing our support have a housing need, we work with a range of housing partners to source suitable properties. Or we may have an existing suitable vacancy.

Accommodation options include:

  • Self-contained apartments with or without communal areas. 
  • Shared supported living houses. 
  • Bespoke properties designed and adapted to meet individual needs e.g. wheelchair accessible, integrated assistive technology.
