Who we are

The Community Connecting team assists customers with a disability to discover more about services in their local community. We will assist you to find and take part in your chosen community activity and to do this independently. The team will help you to find voluntary work, further education, including college courses and other training opportunities, or join a local club and meet new people.

Who we support

  • People 14-25 years old with a statement of Special Education Need (SEN) or who have previously held a statement.
  • People with autistic spectrum condition between 16-25 years of age.

Who is eligible?

  • Young people 14-25 years of age.
  • Young people living in Wiltshire.
  • Someone not working with another Wiltshire Council supported employment service.
  • Someone not supported by a Work Programme.

Who can refer you to the service?

You can be referred to the Community Connecting team by the following contacts:

  • Special Educational Needs or Disabilities (SEND) Worker
  • Wiltshire College Tutor
  • Social Worker
  • Wiltshire Specialist School Teacher
  • Avon and Wiltshire Partnership (AWP), Autism Assessment Team
  • Wiltshire Council; social care advice and Information - 0300 456 0111
  • Integrated Youth Service

What we do

When Community Connecting receives your referral you will be allocated to a Community Co-ordinator, who will register you to our service. Your Community Co-ordinator will ask you what type of volunteering or leisure interest you wish to pursue.
We will support you to explore opportunities in your community; work with you to identify your existing skills and help you to develop any new skills. When you have identified what you would like to do, we will plan with you how to get to your chosen activity/voluntary position. We will review our work with you regularly to ensure you are able to sustain your chosen activity/voluntary position.

Meeting new people and developing friendships is a big part of our task. These friendships can develop around numerous activities, for example at gym sessions, fitness classes, swimming, café visits, meeting in each other’s home, cinema visits, pubs, evening classes, and dog walking.
We would support these friendships in the same way as voluntary positions and activities. Once you have settled into your friendship group your Community Co-ordinator will have a chat with you to review it and leave you to it.

What is ‘Preparing for Your Community?’

We are delivering community focused sessions to students with disabilities within Wiltshire Schools and Colleges. This is a four-week programme that focuses on preparation for your community. The sessions include some of the following:

  • Your Community
  • Hobbies and Interests
  • Personal Safety
  • Travel
  • Friendships
  • Volunteering
