What is a Review?

A review is a conversation, usually face to face but can be by telephone. It is important as it looks at the care and support someone is receiving and checks whether it is still meeting their needs and is right for them.

Reviews can be completed by the Council, organisations providing support services or the person who needs the care and support services.

 

What will Happen?

At the review, the reviewer will work with you to identify how your support plan is helping you to reach your goals (outcomes). This may lead to a re-assessment if it becomes apparent that your needs may have changed during the period of the support plan under review. There may also be times when it will be appropriate to review a support plan without re-assessing your needs.

 

How often will my care package be reviewed?

Reviews usually take place a few weeks after support is arranged and then annually. However you can request a review at any time if things have changed.
